Late intracochlear MRI-evaluated effects after cochlear implantation

Introduction:

Recent developments regarding cochlear implant magnets (e.g., a bipolar diametral magnet) and refined surgical technique (e.g., implant positioning) have made a significant impact on the relation between a cochlear implant and MRI. MRI scanning has changed from a contraindication to a diagnostic tool. For the first time, an pain free in vivo evaluation of the fluid state of the cochlea, following the insertion of an electrode, has become possible via MRI scanning.

The aim of the study was to evaluate late MRI changes of the cochlea following a cochlear implantation by MRI sequences at 3 T.

Methods:

Our study considered 20 patients whom had been implanted with a diametrically bipolar implant magnet system with a lateral wall electrode. We compared the MRI scan of the first postoperative day in an axial and coronar position with a T2 and T1 weighted sequence at 3 T and a resolution of 0.8 mm with the MRI T2 and T1 scan one year after cochlear implantation. Additionally a comparison with the non implanted side was performed.

Results:

In all case an occurring postoperative T2 signal persists even after one year. Comparing the early and late MRI scans an increase of the T2 weighted signal over time could be observed. T1 signal showed no significant signs of a bleeding.

Conclusion:

Late MRI observation provides information about the intracochlear fluid state after a cochlear implantation.
