Untreatable Neuropathic Back Pain: Epidural Electrode OImplant with Neurophysiologic Perioperative Monitorization Using Microscope

Peripheral neuropathic pain is very hard to treat and can be resistant to multiple pain management modalities. The criteria for use of Spinal Cord Electrode Stimulator (SCS) for specific problems such as failed back surgery syndrome (FBSS), A 73 years-old woman with a chronic back pain for the last years reached us in our service to stablish another kind of treatment for her refratary neuropathic pain condition. An Epidural Electrode implant with neurophysiologic perioperative monitorization (somatosensory evoked potential and electromyoprahy), using microscope to treat chronic neuropathic back pain. Multiples spinal cord stimulation (SCS) modes are used in the treatment of chronic neuropathic pain disorders. Spinal cord stimulation provides analgesia through electrical stimulation of the dorsal column of the spinal cord via electrode leads placed into the epidural space. In our case, the patient went home 3 days after the procedure and after 6 months, noticed an important pain relief which increased substantially the quality of life, therefore other studies are needed to improve the technic to pain relief by Epidural Eletrical Stimulator (EES).
