Resumen

La parálisis braquial obstétrica (PBO) es una grave entidad clínica con muchas cuestiones por resolver. Se desconocen los verdaderos factores de riesgo, y aunque la cirugía nerviosa ha mejorado su técnica y las secuelas se previenen con mayor seguridad que hace unos años, el resultado no es el deseado, pues la incidencia de la enfermedad no ha variado y con la cirugía sigue habiendo un porcentaje alto de fracasos en lesiones extensas o que afecten a la mano. En este artículo, se revisará el tratamiento actual en niños con PBO menores de 2–3 años. Se analizarán las técnicas más actuales como son las transferencias nerviosas, la artroscopia de hombro o el uso de la toxina botulínica.

Abstract

The obstetric brachial palsy (OBP) is a serious clinical entity with many questions still to be solved. Its real risk factors are not known yet, and, although nerve surgery technique has improved, and its consequences are better prevented than years ago, the result is not the desired one. In fact, the illness incidence has not changed and surgery still presents a high failure rate in extensive injuries or affecting the hand. In this article it is reviewed the current treatment in children under two-three years with OBP. The most up-to-date techniques, such as the nerve transfers, shoulder arthroscopy or the use of botulinum toxin, will be analyzed.
