Therapeutische Embolisation bei Erkrankungen des zentralen Nervensystems

Zusammenfassung

Die Arbeit befaßt sich mit einigen Möglichkeiten der interventionellen Neuroradiologie vor allem auf dem Gebiet der therapeutischen Embolisation bei Erkrankungen des zentralen Nervensystems.

Die wesentliche Aufgabe dieser Methode besteht darin, durch präoperative Ausschaltung des Tumorkreislaufs bei den gefäßreichen Geschwülsten die Operationsrisiken zu reduzieren. Bei einigen dieser Tumoren der Schädelbasis, wie Glomustumoren, ist eine radikale Operation erst durch die vorangegangene Embolisation möglich geworden. Bei anderen Erkrankungen, wie den duralen Fisteln, ausgedehnten Angiomen und den sonst nicht operablen Riesenaneurysmen, kann die Embolisation als definitive therapeutische Maßnahme angesehen werden und die offenen risikoreichen Operationen überflüssig machen.

Die interventionelle Neuroradiologie hat in den letzten Jahren vor allem durch die Einführung der intraarteriellen digitalen Subtraktionsangiographie und durch die Entwicklung der Mikrokatheter wesentliche Fortschritte erzielt.
