Zusammenfassung

Die chronische Depression ist eine schwer beeinträchtigende psychische Störung, welche meist schon während der Kindheit durch traumatisierende Beziehungserfahrungen beginnt und aufgrund ihrer Therapieresistenz eine große Herausforderung darstellt. Das Cognitive Behavioral Analysis System of Psychotherapy (CBASP) ist die einzige Psychotherapie, welche speziell für diese Patientengruppe entwickelt wurde. CBASP kombiniert schulenübergreifend behaviorale, kognitive, interpersonelle und analytische Strategien. Kindheitsund Lebenserfahrungen werden in einen Zusammenhang zu aktuellen interpersonellen Problemen gestellt. Zentrale Elemente sind die Situationsanalyse (SA) mit Verhaltenstrainings und Diskriminationslernen durch Rollenspiele, interpersonelle Strategien und Transferübungen. Die therapeutische Rolle ist gekennzeichnet durch das disziplinierte Preisgeben eigener Gefühle und Reaktionen ausgerichtet auf die Bedürfnisse chronisch Depressiver, um dem Patienten eine korrigierende und heilsame Beziehungserfahrung zu ermöglichen. In ersten Studien konnte die Wirksamkeit von CBASP in der Behandlung der chronischen Depression nachgewiesen werden. Weitere Studien zum Vergleich mit anderen Therapieverfahren sind notwendig und sollen in den nächsten Jahren folgen.

Summary

Chronic depression is a severe and disabling disorder often beginning in early childhood due to traumatic experiences in interpersonal relationships. Because most of the patients are treatment resistant, chronic depression appears as a demanding therapeutical challenge. The Cognitive Behavioral Analysis System of Psychotherapy (CBASP) is the only psychotherapy developed specifically for this subgroup of patients. CBASP is a school-overlapping method that combines behavioural, cognitive, interpersonal, and analytical strategies. Childhood- and life-experiences are put into relation with current interpersonal problems. Main treatment elements are the situational analysis (SA) with practice of behaviour, interpersonal strategies, cognitive restructuring, and transfer exercises. The therapist’s role is characterized by the disciplined disclosure of own feelings and reactions according to the needs of chronic depressive patients to enable corrective and healing interpersonal experiences. First trials have proven the efficacy of CBASP in the treatment of chronic depression. Further studies, especially in comparison to other treatment strategies, are needed and ongoing.
