Wirbelkörperkompressionsfraktur durch epileptische Anfälle

Zusammenfassung

Wirbelkörperfrakturen sind eine mögliche, häufig aber übersehene Folge generalisierter tonisch-klonischer Anfälle. Wir berichten von zwei Patienten, die die Einnahme ihrer antiepileptischen Medikation beendeten und daraufhin epileptische Anfälle mit assoziierter Wirbelkörperfraktur erlitten. Aufgrund der häufig fehlenden neurologischen Defizite sollte speziell bei postiktalem Rückenschmerz an eine mögliche Wirbelkörperfraktur gedacht werden und eine radiologische Diagnostik erfolgen.

Summary

Vertebral fractures are possible but often not diagnosed consequences of generalised tonic-clonic seizures. We report on two patients with chronic epilepsy treated with antiepileptic drugs. Both patients discontinued their medication abruptly and developed seizures which caused compression fractures of a vertebra. Diagnosis of fractures is often missed because usually neurological investigation remains normal. In case of post-ictal back pain an X-ray examination should be done to rule out a fracture caused by the seizure.
