Diagnostic Value of Routine Liver Scanning in Ecchinococcus Disease of the Liver; a Comparison of Scintigraphic and Surgical Findings in 100 Consecutive Cases

Summary

The diagnostic value of liver scanning for hydatid cysts of the liver was examined by comparing the preoperative scintigrams and the surgical findings in 100 consecutive cases operated upon for hydatid cysts of the liver. False results were found in 10 cases (false positive in 1 and false negative in 9 cases). In the remaining 90 cases the scans had demonstrated the presence of pathological findings supporting the diagnosis of hydatid cysts. In 66 of these 90 cases the number, location and size of the cyst(s) were accurately defined by the scan, while in the remaining 24 cases there was a discrepancy between the scan and the surgical findings as to the number of the cysts. The effect of the number and location of the cysts on the accuracy of the scan diagnosis was analyzed.

On a examiné la valeur diagnostique de la scintigraphic comme mesure de routine pour détecter des kystes hydatiques du foie, en comparant les scintigrammes préopératoires aux résultats chirurgicaux, obtenus de 100 cas consécutifs opérés sur des kystes hydatiques. Des résultats diagnostiques 10 en étaient faux (dont un erronément positif et neuf erronément négatifs). Les scintigrammes des autres 90 patients avaient prédit la présence de kystes. Dans 66 de ces cas, la scintigraphic avait établi un diagnostic exact par rapport au nombre, à la position et à l’extension du/des kyste/s; 24 fois, par contre, une divergence entre les scintigrammes et les résultats chirurgicaux s’était manifestée en ce qui concerne le nombre des kystes. On a analysé l’effet du nombre et de la position des kystes sur la précision du diagnostic scintigraphique.

Durch Vergleiche von präoperativen Szintigrammen mit Operationsergeb-nissen bei 100 konsekutiven Fällen, die wegen Ecchinokokkuszysten der Leber operiert worden waren, wurde der diagnostische Wert der Leberszintigraphie zur Feststellung solcher Zysten untersucht. Bei 10 Patienten wurden falsche Ergebnisse erhalten (falsch-positiv bei einem, falsch-negativ bei 9 Fällen). Bei den restlichen 90 Patienten wies das Szintigramm auf das Vorliegen von Zysten hin. Bei 66 dieser 90 Patienten war die Zahl, der Ort und die Größe der Zyste(n) im Szintigramm genau feststellbar, während bei den verbleibenden 24 Fällen sich eine Diskrepanz zwischen dem Szintigramm und den chirurgischen Befunden bezüglich der Anzahl der Zysten ergab. Es wurde die Auswirkung der Zahl und der Lage der Zysten auf die Genauigkeit der szintigraphischen Diagnose analysiert.
