Resumo
É descrito o tratamento de uma paciente de 28 anos, que apresentava fratura vertebral
cervical em dois níveis (fratura explosão de C6 e subluxação rotatória C1-C2). A massa
lateral direita e a lâmina de C1 apresentavam-se fraturadas. Havia sido feita a traqueostomia
precoce durante a admissão pelo grupo de emergência, o que dificultou o acesso pela
via anterior. Foram realizadas a estabilização posterior C4-T1, com fixação segmentar
(para estabilizar a lesão de C6) e a artrodese C1-C2 com o parafuso transarticular,
apenas em uma articulação, devido a fraturas na articulação C1-C2 contralateral. O
objetivo foi o de estabilizar a coluna espinhal, para movimentação e cuidados respiratórios,
e preservar a motilidade occipto-C1,C2-C3 e C3-C4.
O controle, após 9 meses, mostrou estabilidade atlanto-axial mantida, com fusão interarticular,
comprovando que, neste caso, a técnica transarticular unilateral foi suficiente para
a fixação interna.
Abstract
We describe the surgical treatment of a 28 year-old female patient, who presented
two cervical vertebral fractures (burst fracture of C6 and atlanto-axial rotatory
dislocation).The right lateral mass and the lamina of C1 were fractured. The emergency
attendant physicians had performed a tracheotomy, which made the anterior approach
difficult. We performed a posterior stabilization (C4-T1) with segmental fixation
(in order to stabilize the C6 fracture) and C1-C2 arthrodesis with transarticular
screw on one side only, because of the presence of fractures in the lateral mass on
the other side. The aim was to stabilize the spine for early rehabilitation, respiratory
care and preserve the occiptal-C1, C2-C3 and C3-C4 motility.
The follow-up, after nine months, showed articular stability, with interarticular
fusion.The unilateral transarticular technique was able to maintain the internal fixation.
