Zusammenfassung

Aufgrund des seltenen Vorkommens von Densfrakturen in der Veterinärmedizin wird eine Literaturübersicht über im Schrifttum vorliegende Fallberichte gegeben. Ferner erfolgt die Präsentation eines eigenen Falls. Dem Autor lagen 19 Veröffentlichungen vor, in denen 34 Densfrakturen beim Hund beschrieben wurden. Bei den in 16 Fällen klassifizierten Frakturtypen handelte es sich um acht Densfrakturen Typ III, sieben Densfrakturen Typ II und eine nicht von diesem Autor nicht als solche gekennzeichnete Densfraktur Typ I. Entsprechend dem zugrunde gelegtem Schrifttum stellt die vorliegende Arbeit demnach die erste Fallbeschreibung in der Tiermedizin dar, in der eine Fraktur des Dens axis nach der Klassifikation von Anderson und D‘Alonzo (1) dem Frakturtyp I zugeordnet wurde. Die beschriebene Densfraktur wurde chirurgisch durch eine ventrale Stabilisierung des atlantoaxialen Gelenks mit zwei Zugschrauben versorgt. Eine mit der Entfernung des Gelenkknorpels einhergehende Spondylodese unter Verwendung von autologer Spongiosa erfolgte nicht. Der Hund zeigte 12 Wochen nach Entlassung keine zervikale Dolenz mehr. Sechs Monate post operationem wurde ein beidseitiger Implantatbruch festgestellt, der allerdings ohne klinische Relevanz blieb. Es wird ein Einblick in die teilweise sowohl in der Human- als auch in der Veterinärmedizin kontrovers diskutierten therapeutischen Möglichkeiten der verschiedenen Densfrakturen gegeben. Die abschließenden Schlussfolgerungen, die sich aus der Literatursichtung und den Erfahrungen des Autors bei der Therapie der Densfraktur Typ I ableiten, sollen für den behandelnden Tierarzt grundlegende Entscheidungshilfen für die Wahl der richtigen Therapieform darstellen.

Summary

Due to the rare occurrence of odontoid fracture in veterinary medicine a literature survey of the presented case studies in literature is given. Futhermore an own case is presented. Nineteen publications were reviewed by the author in which 34 odontoid fractures of the dog were described. In 16 patients the fractures were classified according to the classification of Anderson and D’Alonzo (1). These cases comprised eight odontoid fractures type III, seven odontoid fractures type II, and one odontoid fracture type I that had not been identified as such by the author. Based on the literature survey our study therefore presents the first case report in veterinary medicine in which an odontoid axis fracture was assigned to the fracture type I according to the classification of Anderson and D’Alonzo (1). The odontoid fracture described was surgically treated by a ventral atlantoaxial stabilisation of the joint with two tension screws. An accompanying spondylosis under application of autologous spongiosa with the removal of the articular cartilage was not performed. Twelve weeks after having been discharged the dog did not show signs of cervical pain. Six months postoperatively an implant fracture on both sides was diagnosed but was not of clinical relevance. An insight into the partially controversially discussed therapeutic options for various odontoid fractures not only in human medicine but also in veterinary medicine is given. The concluding arguments which arise from the literature classification and the author’s experience with the therapy of an odontoid fracture type I should form the basis of the decision support for the attending veterinarian in choosing the appropriate form of therapy.
