Zusammenfassung
Basierend auf der Erkenntnis, dass die INR-Werte (relative Prothrombinzeiten) exponentiell
mit der Wirkkonzentration des Antikoagulans zusammenhängen, wurden für die optimale
Dosierung von Phenprocoumon folgende Werkzeuge für die tägliche Praxis entwickelt:
Ein einfacher (patentrechtlich geschützter) Dosisrechner in Form eines Rechenschiebers
nach dem Prinzip einer Abtastregelung, eine Dokumentationshilfe in Form einer Tabelle
mit Gebrauchsanweisung für die Ersteinstellung und Dosisanpassung, ein Funktionspapier
zur Ermittlung der Zeitspanne, um die das Medikament vor einem geplanten chirurgischen
Eingriff abgesetzt werden muss.
Der echte Nutzen dieser Hilfsmittel muss sich jedoch erst in der Praxis beweisen.
Immerhin erlaubt das technisch einfache Verfahren dem Anwender, es ohne großen Aufwand
mit seinem üblichen Vorgehen bei der Antikoagulation zu vergleichen.
Summary
Based on the realization that the INR values (relative coagulation times) are connected
exponentially with the effective concentration of the anticoagulants it was developed,
for the optimal dosage by Phenprocoumon the following tools for daily practice: A
simple (as a patent protected) dose computer in form of a slide rule according to
the principle of a sampling control. A documentation aid in form of a table with an
operating instruction for the initial dose and the further dose adjustment. A function
paper for the determination of the time interval, around which the drug before a planned
surgical operation must be set off. The real use of these tools must prove itself
however only in practice. The technically simple procedure nevertheless permits the
individual user to compare it without large expenditure with its usual procedure with
the anti-coagulation.
Résumé
En admettant que les valeurs d'INR (temps partiel de prothrombine) sont en connexion
exponentielle avec la concentration réelle des anticoagulants, on a développé l'appareillage
suivant pour la pratique quotidienne du dosage optimal de la phenprocoumone : un simple
ordinateur (évidemment protégé) sous forme d'une règle de calcul adaptée au contrôle
d'échantillons. Une documentation est fournie pour expliquer la manipulation lors
du dosage initial et pour l'ajustement des dosages suivants. Cela doit entraîner une
information pour la détermination du délai à observer pour l’arrêt du médicament lorsqu’une
intervention chirurgicale est prévue. L'utilité véritable d'un tel appareil ne peut
faire ses preuves que dans la pratique courante. Néanmoins, la simplicité de la manipulation
technique doit permettre à l'utilisateur d'effectuer des comparaisons avec les dosages
habituels, à moindres frais.
