A Study of 408 Cold Thyroid Nodules in a Country with Endemic Goitre

Summary

408 cases of single thyroid nodules, scintigraphically cold, were operated and examined histologically. Malignancy was proved in 53 (13%) of them. The incidence of malignant neoplasms was twice higher in males (23.4%) than in females (11.6%), and four times higher in persons more than 60 years old (46.4%) than in the younger age groups. It is concluded that the incidence of malignancy in single cold nodules of the thyroid is high, even in centres drawing most of their case material from regions with endemic goitre, and for this reason surgical removal is recommended.

408 cas de nodules solitaires de la glande thyroïde, prouvées froides au scintigramme, ont été opérés. A 1’ examen histologique 53 cas (13%) de tumeurs malignes ont été constatés. La proportion de malignité a été deux fois plus haute chez les hommes (23.4%) que chez les femmes (11.6%), et quatre fois plus haute chez les sujets âgés de plus de 60 ans (46.4%) que chez les sujets d’ un âge inferieur. On conclut que la proportion des néoplasmes malins chez les porteurs de nodules solitaires froides est assez haute, même dans des centres où la plupart des cas proviennent d’une zone d’endémie goitreuse, et que l’ opération doit être recommandée régulièrement.

408 Patienten, die szintigraphisdh einen kalten, solitären Schilddrüsenknoten aufwiesen, wurden operiert und der Knoten histologisch untersucht. In 53 Fällen (13%) wurde Malignität festgestellt. Die Häufigkeit von malignen Neoplasmen war doppelt so hoch bei den männlichen Patienten (23,4%) wie bei den weiblichen (11,6%) und viermal so hoch bei über 60 Jahre alten Patienten (46,4%). Es wurde festgestellt, daß auch in Zentren, in denen der größte Teil der Patienten aus Gebieten mit endemischem Kropf stammt, die Malignitäts-Häufigkeit in kalten solitären Schilddrüsenknoten hoch genug ist, um ein chirurgisches Eingreifen zu rechtfertigen.
