Zusammenfassung

Die Gerontorheumatologie befasst sich mit den Besonderheiten rheumatischer Erkrankungen bei Patienten ab dem 60. Lebensjahr. Es handelt sich entweder um entzündlichrheumatische Erkrankungen mit Erstmanifestation im höheren Lebensalter oder um be-sondere Verlaufsformen rheumatischer Erkrankungen bei Patienten, die dieses Lebens-alter erreichen. Wichtigste entzündliche Sys-temerkrankungen des älteren Patienten sind die Polymyalgia rheumatica, die Riesenzell -arteriitis und die rheumatoide Arthritis des älteren Patienten (late onset rheumatoid arthritis, LORA). Die Diagnose wird durch die Häufigkeit begleitender Allgemeinsymptome und die oft uncharakteristische rheumatische Initialsymptomatik erschwert. Da ältere Pa-tienten durch entzündliche Rheumaformen schneller in ihrer Funktionsfähigkeit und Selbstständigkeit bedroht sind als jüngere Pa-tienten, ist eine konsequente adäquate entzündungshemmende Therapie wesentlich. Dabei können prinzipiell alle wesentlichen Basistherapeutika auch im höheren Lebensalter eingesetzt werden, erfordern jedoch eine sorgfältigere Überwachung durch den Rheumatologen.

Summary

Gerontorheumatology covers the clinical spectrum of rheumatic diseases in patients older than 60 years. It includes inflammatory rheumatic diseases which develop in older patients as well as specific manifestations of such diseases when they began earlier but the patients become older. The most important rheumatic diseases in older patients are polymyalgia rheumatica, giant cell arteriitis and late onset rheumatoid arthritis (LORA). The diagnosis is complicated by the frequency of unspecific signs and symptoms such as fever, general malaise or inappatence. Disability and reduction of self competence in activities of daily living may occur early in older patients, due to preexisting reduction. Adequate anti-inflammatory therapy is therefore essential. In general, all disease modifying antirheumatic drugs available may be used in the older population, even though consequent safety evaluations are necessary.
