Zusammenfassung
Die weltweit rapide ansteigende Inzidenz der morbiden Adipositas hat Implikationen
für die Dermatochirurgie. Neben einem erhöhten OPRisiko und OP-Aufwand sind bei Adipösen
dermatochirurgisch zu behandelnde Krankheiten wie z. B. die Acne inversa und das Ulcus
cruris venosum häufiger. Die Liposuktion ist primär zur ästhetischen Therapie diätresistenter
Fettpolster beim Gesunden indiziert. Sie ergänzt die klassischen dermatochirurgischen
Verfahren in der medizinischen Therapie von Krankheiten des Fettgewebes z. B. Lipomatosen,
Lipodystrophien und Lipödem. Als Bodycontouring ist sie im Gesamtkonzept der postbariatrischen
plastischen Chirurgie etabliert. Ob die Liposuktion alleine einen positiven Effekt
auf die metabolischen Komplikationen morbider Adipositas hat, wird kontrovers diskutiert.
Die faziale HIV-assoziierte Lipoatrophie wird mittels Lipotransfer erfolgreich behandelt.
Kosmetische Operationen am Fettgewebe sind die häufigsten ästhetischen Eingriffe weltweit.
Deshalb wird das Risikoprofil der Liposuktion wie auch der sogenannten Lipolyse erörtert.

Summary
The worldwide rapidly increasing incidence of obesity has implications for cutaneous
surgery. In addition to the increased general risk of complications, adipose patients
suffer more often from skin problems requiring dermatologic surgery, particularly
acne inversa and venous leg ulcers. Liposuction is generally performed for cosmetic
reasons. The aim is the reduction of localized fatty tissue in healthy patients, but
medical indications are playing an increasing role: lipomatosis, lipodystrophies and
lipedema. Liposuction as body contouring procedure is an integral part of postbariatric
surgery. Nevertheless it is still controversially discussed whether abdominal liposuction
is a potential treatment for the metabolic complications of obesity. Lipotransfer
is a well-established treatment modality for facial HIV-associated lipoatrophy. Among
cosmetic surgery options, liposculpture is one of the most popular procedures. Risk
profiles of liposuction and lipolysis are discussed.
