Zusammenfassung

Die perkutane Vertebroplastie (PVP) ist ein schmerztherapeutisches Verfahren, bei dem durchleuchtungsgesteuert Knochenzement in einen frakturierten Wirbelkörper injiziert wird. Indiziert ist die PVP bei therapieresistenten, schmerzhaften osteoporotischen Wirbelsinterungen und bei schmerzhaften Wirbeltumoren. Wichtig ist die Durchfüh-rung durch geschultes Fachpersonal an einem Wirbelsäulenzentrum unter Zuhilfenahme einer hochauflösenden Durchleuchtungseinheit und eines vertebroplastiegeeigneten Zements. Schwerwiegende Komplikationen können so weitestgehend vermieden werden. Zur korrekten Indikationsstellung gehören eine gründliche orthopädisch-neurologische Untersuchung sowie die adäquate präoperative Bildgebung mittels Nativ-Röntgenaufnahmen und Kernspintomographie. Frische, behandlungsbedürftige Wirbelsinterungen können dadurch eindeutig identifiziert werden. Bei Beachtung dieser Qualitätsanforderungen wird bei über 80% der behandelten Patienten eine rasche und deutliche Schmerzreduktion erzielt.

Summary

Percutaneous vertebroplasty (PVP) is a treatment for painful osteoporotic vertebral compression fractures refractory to conservative therapy and for painful vertebral tumours. During PVP bone cement is injected into the fractured vertebral body under fluoroscopic guidance. It is essential that this procedure is performed by trained medical specialists in a spine center using a high resolution fluoroscopy unit and a bone cement suitable for PVP. Thus major complications can be avoided as far as possible. A thorough orthopedic and neurological examination is needed as well as adequate preoperative imaging using conventional X-rays and MRI to identify acute vertebral fractures requiring treatment. By following these quality guidelines a fast and significant pain reduction can be achieved in more than 80% of patients treated with PVP.
