Zusammenfassung
Eine operative Therapie an der Wirbelsäule bei axialen Formen der Spondylarthritis ist in folgenden Situationen indiziert: – HWS-Instabilitäten bei jüngeren Patienten, – pathologische Frakturen der Wirbelsäule bei erwachsenen Patienten mit bereits steifer Wirbelsäule nach Bagatelltraumen und schmerzhafter, rascher Zunahme der Deformität, – schwere kyphotische Einsteifung der Wirbelsäule mit Verlagerung des Sichthorizontes und Beeinträchtigung der Fortbewegung und der Atmung, – schmerzhafte progrediente posttraumatische Andersson-Läsionen.
Summary
Surgical treatment of spine instabilities and deformities in SpA patients may be considered in certain situations as follows: – Atlanto-axial dislocation in juvenile patients, – pathologic fractures in adult patients, showing painful rapid deterioration of their cyphotic deformity, – longstanding severe kyphotic deformity of the spine like chin-on-chest deformities or severe thoracic kyphosis with sagittal inbalance of the whole body may reduce the locomotion and breathing excursions of the patient and should be treated surgically, – posttraumatic spondylodiscitis (Andersson lesion).
