Zusammenfassung
1991 haben wir die Resultate der Varizenoperation beim Lymphödem anhand einer Nachkontrolle
von 42 vor 20 Jahren operierten Extremitäten publiziert. Insgesamt konnten die Resultate
als teilweise befriedigend bezeichnet werden. In der Literatur fehlt eine vergleichbare
Studie bis heute. Im folgenden Aufsatz diskutieren wir diese nun schon längere Zeit
zurückliegenden Resultate im Lichte der heutigen fortgeschrittenen Diagnostik und
Operationstechnik. Auch heute kann man höchstens einen Teilerfolg erwarten. Unabdingbare
Voraussetzungen für eine sinnvolle Operation sind die eindeutig phlebogene Symptomatik,
ein umfas-send orientierter und zur vollen Kooperation fähiger Patient und eine gezielte
minimalinvasive Operationstechnik durch einen geübten, sich der Schwierigkeiten bewußten
Chirurgen.
Summary
In 1991 we have published the results of varicose vein surgery in the presence of
lymphedema in 42 legs operated on average 20 years before. In summary the results
were partially satisfactory. For the meantime we have not been able to find a comparable
study in the literature. In this essay the results are discussed in the light of present
day refined diagnostic and operative technique. Even today we cannot expect more than
a partial success. Prerequisites for satisfactory results are that the symptoms are
caused by the phlebologic component of the disease, a comprehensively informed patient
able to cooperate fully, and a limited minimal invasive operative technique by a cautious
and experienced surgeon.
Résumé
En 1991, nous avons publié les résultats d’un contrôle effectué 20 ans après des opérations
de varices chez des patients souffrant conjointement de lymphoedème, l’étude portant
sur 42 membres inférieurs. D’une manière générale, on peut estimer les résultats comme
partiellement satisfaisants. Jusqu’à ce jour, une telle étude n’existe pas ailleurs.
Ces résultats déjà anciens sont maintenant remis à la lumière de l’évolution actuelle
des techniques diagnostiques et chirurgicales. Pourtant, même aujourd’hui, on ne pourrait
espérer mieux qu’un succès partiel. Les conditions indispensables à une intervention
raisonnable sont: une symtomatologie nettement phlébogène, une claire orientation
et une totale collaboration du patient, ainsi qu’une technique opératoire mini-invasive
de la part d’un chirurgien expérimenté et ayant le sens de la retenue.
