Extended Axillary Flap: Effective Pedicle Flap for Anterior Skull Base Reconstruction

Introduction: There are different surgical techniques for skull base reconstruction that had been recommended by many authors. These strategies give the skull base surgeon many options depending on pre- and intraoperative conditions. Due to challenging and complexity of skull base pathology, innovation of different surgical option to protect the neurovascular structure are compelled. We present an extended axillary flap for anterior skull base reconstruction that based on posterior pedicled.
Method: A cadaveric study on anatomy laboratory had been performed to illustrate the feasibility on operative room for endoscopic extended axillary flap for anterior skull base reconstruction. The measurement was taken and then applied on an elderly lady with left nasoethmoidal encephalocele.
Result: Another option for skull base surgeon for endoscopic anterior skull base reconstruction was discussed and inspected. The healing after the surgery was excellent and patient recovery very well. No evidence of CSF leak after surgery.
Conclusion: We present another option for endonasal skull base reconstruction which can give the surgeon flexibility to utilize mucosal flap if the other flap cannot be utilized.
