Herein we describe a novel safe-entry zone, the superior fovea triangle, on the floor of the fourth ventricle for resection of deep dorsal pontine lesions at the level of the facial colliculus. We present clinical data from patients undergoing a suboccipital, telovelar, trans-superior fovea triangle approach to deep pontine lesions. This clinical information is supplemented with anatomical descriptions using fiber dissection to define the boundaries of this safe zone.
