Rhabdomyosarcoma of the embryonal type is the most common mesenchymal malignant tumor in pediatric age group. However it is rare in adult more than 40 years. Patients under 2 years of age tend to have less advanced disease, and when the disease arises in the parameningeal site such as middle ear or orbit, direct meningeal extension may occur. We described a case of 50 year-old lady with progressive embryonal rhabdomyosarcoma of the left orbit and ethmoidal sinus which extend to the anterior cranial fossa after completed combination of chemotherapy and surgery.
