Zusammenfassung

Einleitung: Im letzten Jahrzehnt hat sich die endovaskuläre Therapie von Aortenaneurysmen (EVAR) durch die medizintechnische Weiterentwicklung zum Standardeingriff entwickelt und wird flächendeckend durchgeführt. Auch komplexe anatomische Verhältnisse stellen oft keine Kontraindikation mehr dar. Ein integraler Bestandteil des minimalinvasiven Verfahrens ist die Bildgebung vaskulärer Strukturen zur exakten Platzierung der Stentprothesen. Derzeit erfolgt die intraoperative Darstellung der Gefäßanatomie durch eine 2-dimensionale Angiografie mit Kontrastmittelapplikation und Durchleuchtung, welches hohe Anforderungen an den endovaskulären Chirurgen stellt und Erfahrung verlangt. Material und Methode: Wir berichten über einen Prototyp zur Echtzeitnavigation während der EVAR-Prozedur mit Darstellung einer virtuellen Angioskopie des Gefäßsystems bei Patienten mit infrarenalem Bauchaortenaneurysma. Hierzu erfolgt aus der präoperativ angefertigten Dünnschicht-CT-Angiografie die 3-D-Rekonstruktion und Gefäßsegmentierung in einem Volumendatensatz. Durch Registrierung wird ein Live-Tracking während der Stentprothesen-Positionierung ermöglicht. Zu Beginn der Intervention werden relevante Landmarken des segmentierten Volumendatensatzes mit dem 2-dimensionalen Angiografiebild abgeglichen. Während der Intervention erfolgt die kontinuierliche Registrierung der Position des Katheters bzw. Stentgrafts. Zur Visualisierung wird eine 3-dimensionale endoluminale Darstellung im Sinne einer virtuellen Angioskopie generiert. Ergebnisse: Wir untersuchten die Zusammenführung von Hard- und Softwarekomponenten inklusive komplexer Bildregistrierungsverfahren und faseroptischer Sensorik (Fiber-Bragg-Navigation) mit Integration in Stentprothesen-Einführsysteme im experimentellen Setting. Die medizintechnische „Feasibility“ einer glasfaserbasierten Navigation wurde in unseren experimentellen Versuchen an patientenindividuellen Gefäßphantomen belegt. Die 3-dimensionale präoperative Planung mit Registrierung und Simulation einer virtuellen Angioskopie in Echtzeit wurde konkret umgesetzt. Schlussfolgerung: Ziel des Nav-CARS-EVAR-Konzepts ist die Reduzierung von Kontrastmittel- und Strahlendosis durch eine 3-dimensionale Navigation während der EVAR-Prozedur. Zur Implementierung der Fiber-Bragg-Navigation sind vor der klinischen Anwendung weitere experimentelle Untersuchungen zur Verifikation der Genauigkeit notwendig.

Abstract

Introduction: Over the last decade endovascular stenting of aortic aneurysm (EVAR) has been developed from single centre experiences to a standard procedure. With increasing clinical expertise and medical technology advances treatment of even complex aneurysms are feasible by endovascular methods. One integral part for the success of this minimally invasive procedure is innovative and improved vascular imaging to generate exact measurements and correct placement of stent prosthesis. One of the greatest difficulty in learning and performing this endovascular therapy is the fact that the three-dimensional vascular tree has to be overlaid with the two-dimensional angiographic scene by the vascular surgeon. Material and Methods: We report the development of real-time navigation software, which allows a three-dimensional endoluminal view of the vascular system during an EVAR procedure in patients with infrarenal aortic aneurysm. We used the preoperative planning CT angiography for three-dimensional reconstruction of aortic anatomy by volume-rendered segmentation. At the beginning of the intervention the relevant landmarks are matched in real-time with the two-dimensional angiographic scene. During the intervention the software continously registers the position of the guide-wire or the stent. An additional 3D-screen shows the generated endoluminal view during the whole intervention in real-time. Results: We examined the combination of hardware and software components including complex image registration and fibre optic sensor technology (fibre-bragg navigation) with integration in stent graft introducer sheaths using patient-specific vascular phantoms in an experimental setting. From a technical point of view the feasibility of fibre-Bragg navigation has been proven in our experimental setting with patient-based vascular models. Three-dimensional preoperative planning including registration and simulation of virtual angioscopy in real time are realised. Conclusion: The aim of the Nav-CARS-EVAR concept is reduction of contrast medium and radiation dose by a three-dimensional navigation during the EVAR procedure. To implement fibre-Bragg navigation further experimental studies are necessary to verify accuracy before clinical application.
