Zusammenfassung
Der Over-the-scope-Clip (OTSC) wird hauptsächlich zum endoskopischen Verschluss gastrointestinaler Defekte oder zur Blutstillung verwendet. Der Einsatz bei der Resektion subepithelialer Tumoren oder gar zur endoskopischen Vollwandresektion ist bislang noch Gegenstand von Untersuchungen. Neuroendokrine Tumoren (NET) des Duodenums sind seltene Erkrankungen, bei denen bis zu einer Größe von 20 mm endoskopisch reseziert werden kann, allerdings aufgrund der subepithelialen Lage Tumorreste zur Tiefe hin verbleiben können. Wir berichten über einen 60-jährigen Patienten mit zahlreichen Vorerkrankungen, bei dem wir nach R1-Resektion (vertikal) eines duodenalen NET (G1) komplikationslos eine OTSC-assistierte tiefe Nachresektion endoskopisch durchführten. Histologisch konnten so Tumorfreiheit (R0) erreicht und eine offene Resektion vermieden werden. Unser Fallbericht demonstriert, dass die OTSC-assistierte tiefe Resektion subepithelialer Tumoren eine mögliche und sichere Alternative speziell für Patienten und Lokalisationen darstellt, bei denen eine operative Entfernung mit einem erhöhten Risiko verbunden ist.
Abstract
The over the scope clip (OTSC) is mainly used for closure of gastrointestinal endoluminal defects and treatment of gastrointestinal bleeding. Its use for resection of subepithelial tumors or full-thickness resection is still under investigation. Duodenal neuroendocrine tumors (NET) are rare neoplasms. Endoscopic resection is appropriate up to a size of 20 mm, however positive deep margins are a frequent challenge in these subepithelial tumors. We report on a 60-year-old male patient who had undergone endoscopic mucosal resection with R1 deep margins of a NET (G1) in the duodenal bulb. To avoid local surgical resection in this multimorbid patient, we performed OTSC-assisted deep resection. Complete resection (R0) was achieved, and no complications occurred. Our report suggests that OTSC-assisted resection of subepithelial tumors is a possible and safe option, especially for patiens and in locations with a high perioperative risk.
