Maldeszensus testis im Kindesalter

Zusammenfassung

Der Maldeszensus testis (MT) stellt mit ca. 3 – 5 % die häufigste Anomalie des Urogenitaltraktes dar. Aktuell werden multifaktorielle Ursachen für den Hodenhochstand diskutiert. Die wichtigste Behandlungsindikation des MT neben Aspekten des Körperbildes sind die Fertilitäts- und Malignitätsprotektion. Therapieziel ist die orthotope skrotale Hodenlage. Ob und inwieweit eine nicht zeitgerechte Behandlung die testikuläre Parenchymfunktion verändert und mit eingeschränkten Ejakulatparametern einhergeht, ist in weiteren prospektiv randomisierten Langzeituntersuchungen zu klären.
Für einen über das 12. Lebensjahr hinaus unbehandelten Hodenhochstand muss ein deutlich erhöhtes Malignitätsrisiko angenommen werden. Da ein spontaner Deszensus bis zum 6. Lebensmonat möglich ist, wird die operative Therapie um den 10. – 12. Lebensmonat empfohlen und kann mit einer Hormontherapie zur möglichen Fertilitätsprotektion kombiniert werden.
Die körperliche Untersuchung in warmer Umgebung dient zur Exploration der Hodenlage und der Differenzierung zwischen Gleithoden, Pendelhoden und einem Kryptorchismus, ggf. wird die Untersuchung durch eine Sonografie ergänzt. In speziellen Konstellationen, bspw. beim beidseitigen, nicht detektierbaren Hoden, kann die Untersuchung durch endokrinologische Labordiagnostik erweitert werden. Bei Verdacht auf eine abdominelle Hodenlage ergibt die diagnostisch-explorative Laparoskopie definitive Klärung.
