Zusammenfassung

Thrombophilie oder Thromboseneigung kann durch genetisch bedingte Defekte wie die Faktor-V-Leiden- und die Prothrombingen-Mutation oder erworbene Störungen, z. B. östrogeninduzierte APC-Resistenz, sowie das Antiphospholipidsyndrom verursacht werden. Die Schwangerschaft greift in erheblichem Maße in das Gerinnungssystem ein, was zu einem weiteren Anstieg des bereits erhöhten Thromboserisikos von Frauen mit einer Thrombophilie in der Schwangerschaft führt. Außerdem wird auch ein Zusammenhang zwischen Thrombophilien und Schwangerschaftskomplikationen wie rezidivierenden Aborten, intrauterinem Fruchttod, Präeklampsie und HELLP-Syndrom in Erwägung gezogen [1] [2]. Als Ursachen für die plazentaassoziierten Schwangerschaftserkrankungen werden dabei eine gestörte Differenzierung des Trophoblasten im 1. Trimenon und eine unzureichende Dilatation der utero-plazentaren Gefäße diskutiert, die infolge einer lokalen Hämostasestörung mit gesteigerter Thrombinbildung durch einen direkten Einfluss von Thrombin auf das Trophoblastzellwachstum entstehen. Ferner können Makro- und Mikrothrombosierungen der fetalen Stammzottengefäße und Spiralarterien zur multiplen Infarktbildung mit Freisetzung von nekrotischen Trophoblastfragmenten und Produktion inflammatorischer Zytokine führen, welche in der Pathogenese von gehäuften Aborten und intrauterinem Fruchttod eine sehr wichtige Rolle spielen [2]. Bei bekannter Thrombophilie wird nach individueller Risikostratifizierung entweder eine postpartale oder eine kombinierte ante- und postpartale Thromboseprophylaxe mit niedermolekularen Heparinen (NMH) empfohlen. Der Nutzen der NMH zur Vermeidung von Schwangerschafts komplikationen wird kontrovers diskutiert. Bei bestimmten Krankheitsentitäten, etwa beim Antiphospholipidsyndrom,

kann eine zusätzliche Behandlung mit Acetylsalicylsäure (ASS) das Schwangerschaftsoutcome verbessern [3] [4]. In diesem Fallbericht werden die Schwangerschaftsverläufe bei einer 28-jährigen Frau mit heterozygoter Prothrombingenmutation und HELLP-Syndrom geschildert, die nach Spätabort und Totgeburt unter einer prophylaktischen antenatalen Antikoagulation mit NMH, kombiniert mit einer engmaschigen interdisziplinären Schwangerschaftsbetreuung, 3 gesunde Kinder zur Welt brachte.

Abstract

Thrombophilia is a prothrombotic state that can be caused by genetic disorders, such as the factor-V-Leiden or prothrombin mutation, as well as by acquired changes like oestrogen-induced APC resistance and the antiphospholipid syndrome. Pregnancy induces multiple procoagulant changes in the haemostatic system, increasing the risk of venous thromboembolism in women with a thrombophilia even further. Additionally, thrombophilias are suggested to be associated with a number of pregnancy complications such as recurrent miscarriage, stillbirth, preeclampsia and HELLP syndrome [5] [6]. Increased local activation of coagulation may directly influence trophoblast expansion and invasion, causing thereby an impaired trophoblast development and insufficient widening of spiral arteries in the first trimenon, which results in placenta-me­diated pregnancy complications like preeclampsia or HELLP syndrome. Besides, macro- and microthrombosis in the vessels of placental stemm villi and spiral arteries may lead to multiple infarctions with release of necrotic trophoblast fragments and inflammatory cytokines playing an important role in the pathogenesis of recurrent pregnancy loss and stillbirth [6]. For women with a known thrombophilia it is recommended to carry out either only postpartal or combined ante- and postpartal thrombosis prophylaxis with low-molecular weight heparins (LMWH) depending on the individual risk stratification. The effectiveness of the LMWH administration for prevention of thrombophilia-induced pregnancy complications and improvement of the pregnancy outcome is currently a matter of debate. Furthermore, an additional application of acetyIsalicylic acid (ASA) should be considered in the management of women with the antiphospholipid antibody syndrome [3] [4]. In the current article we present the case of a 28-year-old woman with the heterozygous prothrombin mutation, HELLP syndrome, a late miscarriage and a stillbirth in the anamnesis, who delivered 3 healthy babies under antenatal LMWH prophylaxis combined with intensive interdisciplinary prenatal care.
