Meningioma is a tumor that developed from arachnoid caps cell and growing at any site inside the cranium, tuberculum sellae meningioma is one of them. Surgery without damaging the brain is the primary goal. One important point in the successful operation of tuberculum sellae meningiomas is surgery approach. There are many approaches in surgery of tuberculum sellae meningioma. Our department uses three approaches to remove the tumor: supraorbital, subfrontal unilateral interhemispheric, subfrontal/frontobasal. The consideration in choosing the approach is based on risk and benefit according to size, position, and enlargement, and attachment to the vital organ such as optic nerve, olfactory nerve, vessel, and hypophysis stalk. Study of the imaging and understanding of the anatomy is an important role to decide in choosing the approach.
