Objective: One of the most commonly encountered vascular compression syndrome is trigeminal neuralgia with an incidence of 4.7 per 100,000. Aided by the ultra-precise features from MRI and through the retrosigmoid approach by the way of combined microscopic and endoscopic procedures, it is regularly possible to cure safely the patients from their pathology. Methods: A total of 70 patients have been operated in our center and we propose to analyze our results. Results: No severe complications were observed and no patient was hospitalized in intensive unit care. All but four patients are cured of their pain and no longer take any medical treatment. One patient has suffered a hearing loss. Conclusion: Neurovascular decompression is the treatment of choice in the treatment of trigeminal neuralgia.
