Zusammenfassung

Kardiochirurgische Eingriffe können mit oder ohne Herz-Lungen-Maschine (HLM) durchgeführt werden. Dabei bestimmt die operative Strategie die Zugangswege der High-Flow-Kanülen für die HLM und die Instrumentierung zur Überwachung (invasive Blutdruckmessung, zentraler Venenkatheter etc.) während der verschiedenen operativen Phasen. Anatomische Kenntnisse des Gefäßsystems sind dabei essenziell, um die Kanülierungswege v.a. bei thorakalen Gefäßeingriffen zu verstehen und die notwendige Instrumentierung adäquat einzusetzen. Der Artikel bietet einen Überblick über mögliche Überwachungsverfahren im Kontext mit operationstechnischen Besonderheiten in der Kardiochirurgie für Anästhesisten und Intensivmediziner.

Abstract

Cardiac surgery requires cardiopulmonary bypass (CPB) with extracorporeal circulation (ECC) for intracardiac procedures. The surgical strategy determines access for monitoring and insertion sites with high-flow cannulas. The perioperative care of cardiac surgical patients requires adequate hemodynamic monitoring for reasonable catecholamine therapy and fluid management. Therefore, the knowledge of the vascular anatomy is essential to provide professional care to patients undergoing ECC during thoracic vascular and cardiac surgery. This article is a review of hemodynamic monitoring and access for ECC in patients for adult cardiac surgery for anaesthesiologists and intensivists.

Kernaussagen

  • Standardmonitoring und -instrumentierung für kardiochirurgische Eingriffe sind:

    • 5-Kanal-EKG,

    • Pulsoxymetrie,

    • invasive Blutdruckmessung,

    • kontinuierlicher zentraler Venendruck,

    • Bilanzierung,

    • regelmäßige arterielle und zentralvenöse Blutgasanalysen und

    • Überwachung der Gerinnung bzw. Antikoagulation.

  • Ergänzende Monitoringverfahren wie z. B. prozessiertes EEG, zerebrale regionale Sättigung (Nahinfrarotspektroskopie, NIRS) und transösophageale Echokardiografie (TEE) können die Patientensicherheit während kardiochirurgischer Eingriffe erhöhen.

  • Die Herz-Lungen-Maschine ermöglicht den kardiopulmonalen Bypass in Hypothermie, ersetzt die Funktion des Herzens und der Lungen und schafft ein blutleeres und bewegungsfreies OP-Feld.

  • Das Blut gelangt über eine venöse Linie in das venöse Reservoir und wird pumpengetrieben durch den Oxygenator transportiert. Das decarboxylierte und oxygenierte Blut wird über die arterielle Linie in das Gefäßsystem des Patienten zurückgegeben.

  • Standardzugänge zum Anschluss an die Herz-Lungen-Maschine sind die venöse Kanüle in den rechten Vorhof und die arterielle Kanüle in die Aorta ascendens.

  • Eingriffe der thorakalen Aorta bedingen je nach anatomischer Ausdehnung die Separation verschiedener Perfusionsgebiete (Truncus brachiocephalicus, A. carotis links und A. subclavia links sowie Aorta descendens) und können eine zweite invasive Blutdruckmessung oder alternative Messverfahren (z. B. NIRS) im entsprechenden Stromgebiet indizieren.
