Rückenschmerz ist eines der häufigsten Krankheitssymptome in unserer Gesellschaft. Je nach Art des Schmerzes, nach dem Zeitpunkt des Auftretens, dem Verlauf und dem Alter des Patienten ist der Arzt vor die differentialdiagnostische Herausforderung gestellt, eine richtige Zuordnung des Schmerzgeschehens zu treffen, um die richtigen Therapiemaßnahmen einzuleiten. Der vorliegende Artikel soll eine Hilfestellung dafür geben, wann aufgrund der Leitsymptomatik eines entzündlichen Rückenschmerzes insbesondere an eine entzündlich-rheumatische Ursache zu denken ist, um nach differentialdiagnostischer Betrachtung, ergänzender laborchemischer und bildgebender Diagnostik eine adäquate Therapie einleiten zu können.

Back pain is a very common symptom in today's society. It is a diagnostic challenge for the physician to decide on the appropriate treatment based on the quality of the pain, the time of its onset, the course of the pain and the age of the patient. The following article is intended to help recognizing an underlying inflammatory rheumatic disease based on the cardinal symptom of inflammatory back pain as well as laboratory tests and diagnostic imaging in order to initiate adequate treatment.
