Juvenile idiopathische Arthritis und Störungen der dentofazialen Entwicklung: Behandlung während der Wachstumsperiode

Zusammenfassung

Die Wachstumsstörungen auf Grund von Entzündungen der Kiefergelenke stellen bei Patienten mit juveniler idiopathischer Arthritis wohl eines der häufigsten Probleme dar. Der Knorpel im Kiefergelenk spielt für die Entwicklung eines normalen Unterkiefers und seiner Beziehungen zum Oberkiefer und anderen Strukturen des kraniofazialen Komplexes eine entscheidende Rolle. Bei juveniler idiopathischer Arthritis hat die Entzündung des Kiefergelenks häufig eine spezielle dentofaziale Anomalie zur Folge. Die frühe Diagnose der Kiefergelenksarthritis ist schwierig und wird häufig erst dann gestellt, wenn bei einem Patienten äußerlich bereits eine deutliche Abweichung erkennbar ist. Eine schwere Störung der dentofazialen Morphologie kann eine eingeschränkte, unangenehme und schmerzhafte Funktion zur Folge haben. Es gibt zwar Behandlungsmöglichkeiten, diese beruhen jedoch ausschließlich auf klinischer und empirischer Erfahrung. Der vorliegende Beitrag beschreibt ein Protokoll zur Behandlung der dentofazialen Anomalie, die sich bei Patienten mit juveniler idiopathischer Arthritis entwickelt.

Abstract

One of the most conferred issues in patients with juvenile idiopathic arthritis is the growth deviation related to arthritis in the temporomandibular joint. The temporomandibular joint cartilage plays an important role in the development of a normal mandible and its relation to the maxilla and other parts of the craniofacial components. In juvenile idiopathic arthritis, arthritis of the temporomandibular joint is frequently leading to a specific dentofacial anomaly. Diagnosis of early temporomandibular joint arthritis is difficult and is often recognized when the appearance of the patient demonstrates a clear deviation. A severe change of dentofacial morphology can lead to disabled and uncomfortable function and pain. Treatment options exist but only based on clinical and empirical experience. The present paper describes the protocol concerning treatment of the dentofacial anomaly developing in patients with juvenile idiopathic arthritis.
