Die neuen ESC-Leitlinien – Was hat sich geändert?

Jeder vierte Schlaganfall wird auf Vorhofflimmern zurückgeführt. Diese Schlaganfälle sind besonders schwer und haben eine ungünstigere Prognose als andere Formen von Schlaganfällen. Der Einsatz von Vitamin-K-Antagonisten ist durch die Besonderheiten ihrer Pharmakologie stark eingeschränkt. Die ESC-Leitlinien vom August 2012 favorisieren daher die Anwendung der neuen oralen Antikoagulantien (NOAKs) weil sie eine bessere Wirksamkeit und Sicherheit bieten und in ihrer Anwendung praktikabler als die Vitamin-K-Antagonisten erscheinen. Bei der Therapieentscheidung sollen jedoch sowohl Patientencharakteristika, Compliance und Verträglichkeit der Medikamente als auch Kostenüberlegungen bei der Entscheidungsfindung berücksichtigt werden. Patienten, die gut auf Vitamin-K-Antagonisten eingestellt sind, können auch weiterhin damit behandelt werden.

Every fourth stroke is attributed to atrial fibrillation. These strokes are particularly severe and have an more unfavorable prognosis than other forms of strokes. The administration of vitamin K antagonists is strictly limited on account of their pharmacological features. The ESC Guidelines dated August 2012 therefore favor use of the new oral anticoagulants (NOACs) because they appear to have better efficacy and safety and are more practical in use than the vitamin K antagonists. In therapeutic decision making, however, the patient's characteristics, compliance, and tolerance as well as cost aspects should be taken into consideration. Patients who are well adjusted to vitamin K antagonists may also continue with this treatment.
