Zusammenfassung

Das Management des chronischen Handekzems stellt oftmals eine Herausforderung für Dermatologen dar. Wenn die Krankheit durch konventionelle lokale Therapiemaβnahmen nicht adäquat kontrolliert werden kann, wird eine systemische Behandlung erforderlich. Wir berichten über eine 78-jährige Patientin, die unter einem schweren psoriasiformen chronischen Handekzem über mehrere Jahre gelitten hat. Die Vorbehandlung beinhaltete topische Kortikosteroide, Fototherapie (308 nm-Excimer-Laser) und Acitretin. Letztlich führte eine Therapie mit 2 Zyklen Alitretinoin (Toctino®) und topischer Erhaltungstherapie mit Tacrolimus 0,1 % zu einer lang anhaltenden Remission.

Abstract

The management of chronic handeczema is often a challenge for the dermatologists. When the disease cannot be controlled by conventional topical therapeutical measures, then a systemic treatment is needed.

We describe the case of a 78-year-old patient who developed over several years a severe chronic psoriasis-like eczema of the hands. The treatment first applied comprised topical steroids, phototherapy (Excimer-Laser 308 nm) and systemic retinoids. Eventually, the treatment with two Alitretinoin (Toctino®)-courses and topical maintenance-therapy with Tacrolimus 0.1 % led to a long-lasting remission.
