Abstract

Emergency echocardiography is helpful to provide an immediate assessment of cardiac morphology and cardiac function in patients with unstable cardiovascular diseases. A structured investigation of all cardiac target structures in non-specific circumstances or a target-oriented investigation due to the suspected diagnosis are the two main approaches of emergency echocardiography. The term emergency echocardiography is reserved for a comprehensive investigation done by a physician who is able to perform the study independently and to interpret the results unaidedly. It should be distinguished from focused cardiovascular echo-scanning performed with pocket-size imaging devices or performed by investigators without a level of clinical competence like a cardiologist.

  • Die Notfall-Echokardiografie ist hilfreich zur schnellen Diagnosestellung und damit zur schnellen Therapieeinleitung bei Patienten mit kardiovaskulären Erkrankungen.

  • Die strukturierte Darstellung von kardialen Zielstrukturen bei unklarer Ausgangssituation und die zielgerichtete Darstellung bestimmter kardialer Strukturen bei bekannter Diagnose oder primärer Verdachtsdiagnose bestimmen die Vorgehensweise bei der Notfall-Echokardiografie.

  • Im Notfall ist es erforderlich, mit dem Echokardiografie-Gerät schnell zu arbeiten. Daher muss der Untersucher mit der Echokardiografie maximal vertraut sein, um mit bestmöglichen technischen Fertigkeiten unter ungünstigsten Bedingungen die besten Bilder mit der besten diagnostischen Aussagekraft erstellen zu können. Damit ist die Notfall-Echokardiografie eine qualifizierte Untersuchung, die eigenständig und eigenverantwortlich von einem Untersucher mit Kardiologen-Status durchgeführt wird.

  • Davon zu trennen sind fokussierte sonografische Untersuchungen, die im Notfall zwar generell hilfreich sein können, jedoch aufgrund einer unzureichenden Gerätetechnik, einer inkompletten Bilddokumentation und einer limitierten Untersucher-Expertise nicht dieser Definition entsprechen. Diese Untersuchungsmodalitäten im Notfall sind als Echo-Scannen bzw. Notfall-Echo-Screening zu bezeichnen.
