Zusammenfassung

Fallberichte sind seit jeher ein zentrales Element der Wissensvermittlung in der Medizin. Zahlreiche medizinische Entdeckungen und Entwicklungen wurden über Fallberichte veröffentlicht. Versierte Kliniker können wichtige Beobachtungen in der Patientenversorgung – dem point of care – über Fallberichte mitteilen. Auch für die Lehre sind Fallberichte unverzichtbar. Die erkenntnistheoretischen Grundlagen des validen Beurteilens im Einzelfall, wie auch generell des klinischen Urteils wurden jedoch bislang kaum systematisch untersucht; hier erweisen sich Prinzipien des Gestalt- basierten Erkennens als ausschlaggebend. Die Publikationsqualität von Fallberichten war in der Vergangenheit vielfach ungenügend; Konsensus-basierte Publikationsleitlinien und Schulung von Ärzten können hier Hilfestellung leisten. Für die systematische Verwendung von Informationen aus Fallberichten werden z.Z. Datenbanken aufgebaut und müssen spezifische Auswertungsmethoden entwickelt werden.

The importance of case reports
Case reports are a vital element of knowledge dissemination in the medical sector. Accordingly, a large number of medical findings and developments have been published as case reports. Thus, case reports have proved to be an important forum for experienced clinicians to publish their observations on patient care (point of care). Case studies are also an indispensable tool in the teaching profession. However, the epistemological foundations for valid judgment of individual cases, as well as of general clinical judgment, have so far not been systematically investigated. In this regard, the principles of Gestalt-based cognition prove decisive. In the past, the publication quality of case reports was often weak; consensus-based publication guidelines and training of doctors can provide guidance for improvement. Currently, structured databases are being established to facilitate the systematic use of information from case reports.
