Zirkonoxidkeramik in der zahnärztlichen Prothetik – eine Standortbestimmung 2013

Einleitung

Vollkeramische Restaurationen aus unterschiedlichen Werkstoffen haben in den letzten 30 Jahren verstärkt Eingang in das restaurative Behandlungskonzept gefunden. Insbesondere die hervorragende Biokompatibilität, aber auch die verbesserte Ästhetik im Vergleich zu metallgestützten Herstellungsverfahren begründeten das verstärkte Interesse an dieser Werkstoffgruppe.

Diesen Vorteilen stand jedoch über lange Zeit ein erhöhtes Risiko technischer Komplikationen bei vollkeramischen Restaurationen gegenüber. Während die Fraktur eines Metallgerüsts insgesamt eine seltene Fehlerursache darstellt, waren Gerüstbrüche bei vollkeramischen Restaurationen lange ein Hauptgrund für den Verlust einer Restauration [1], [2].

Mit der Einführung hochfester Oxidkeramiken (Aluminium- und Zirkonoxid) in der Zahnheilkunde standen Werkstoffe mit deutlich verbesserten Kennwerten zur Verfügung, die eine Verbesserung der Langzeitstabilität vollkeramischer Restaurationen erwarten ließen. Insbesondere die Einführung von Yttrium-teilstabilisierter Zirkonoxidkeramik und erhebliche Fortschritte in der CAD/CAM-Technologie waren Meilensteine für die Indikationserweiterung vollkeramischer Restaurationen auf Brückenversorgungen – auch im Seitenzahnbereich – und ermöglichten in den letzten 10 Jahren die breite Anwendung in der zahnärztlichen Praxis [3] (Abb. [1]).

Entsprechend bieten sich heute für die restaurative zahnmedizinische Versorgung je nach Behandlungskonzept verschiedene Restaurationstechniken an. Die Herstellung einer Krone oder Brücke kann entweder mit einer klassischen metallgestützten oder aber mit einer vollkeramischen Versorgung erfolgen. Für die Auswahl des geeigneten Behandlungsverfahrens ist in diesem Zusammenhang neben Befund und patientenbezogenen Faktoren vor allem die klinische Langzeitbewährung der einzelnen Therapieoptionen zu berücksichtigen.
