Troubleshooting in der lingualen Orthodontie unter besonderer Berücksichtigung einer vollständig individuellen lingualen Apparatur

Zusammenfassung

Die Entwicklung eines vollständig individuellen lingualen Bracketsystems im Jahr 2002 und dessen seitdem fortlaufende Optimierung haben dazu geführt, dass eine große Anzahl von Problemen, welche bei der Behandlung mit lingualen Apparaturen klinisch relevant waren, mittlerweile abgeschwächt und sogar gelöst werden konnten.

Ziel der vorliegenden Arbeit ist es, eine Systematisierung der aktuell noch verbleibenden sogenannten „Troubles“, welche im Rahmen einer lingualen orthodontischen Behandlung mit einem vollständig individualisierten Bracketsystem auftreten können, 1.) zu erstellen und 2.) effektive Wege zu deren Vermeidung bzw. Beseitigung aufzuzeigen und zu erläutern.

Zu diesem Zweck erfolgte eine ausführliche Analyse der klinischen Fachliteratur. E-Learning-Plattformen und klinische Erfahrungsberichte dienten zusätzlich der möglichst vollständigen Zusammenstellung der wichtigsten Komplikationen.

Die Ergebnisse zeigen, dass das Wissen über ein effektives Fehlermanagement bei der Behandlung mit einer vollständig individuellen lingualen Apparatur weit vorangeschritten ist. Bei Beachten der jeweils speziellen Empfehlungen ist die orthodontische Behandlung mit einer solchen Apparatur daher als eine im Praxisalltag sehr effiziente und für den Patienten angenehme Alternative zu der Behandlung mit labialen Bracketsystemen anzusehen.

Abstract

In 2002 a completely individualized lingual appliance was developed and since then its application has been continuously improved. Therefore a variety of clinical problems which had been relevant until then could be reduced, but some of these troubles may still arise during treatment.

Based on the available literature, E-learning material and systemized clinical observations, these actually still possible troubles are presented in a systematic order. Moreover, the best possible solutions for each of these clinical problems are depicted and explained in detail.

The main result of this specified review is that nowadays the treatment with a completely individualized lingual appliance can be considered to be a safe and effective treatment method, if the specific rules how to deal with clinical complications are known and applied. Thus this treatment method seems to fulfill all necessary presuppositions in order to partly replace the more traditional labial treatment methods.
