Zusammenfassung

Multiresistente Erreger (MRSA, VRE und ESBL) stellen in zunehmendem Maße ein Problem für medizinische Einrichtungen dar. Unter den HA-MRSA(health care associated MRSA)-Trägern gilt eine < 12 Monate zurückliegende endoskopische Untersuchung als ein möglicher Übertragungs­weg. Neben Bronchoskopien besitzt die perkutane endoskopische Gastrostomie wegen der oro-pharyngealen Besiedelung ein beträchtliches Kontaminationsrisiko. Da die Keime in der Umwelt lange persistieren, sind bei Kontakt mit MRSA-Patienten in endoskopischen Ein­richtungen besondere Präventionsmaßnahmen zu beachten. Neben allgemeiner Dekontamination gelten für das ärztliche und Assistenzpersonal Kittel, Mund-, Nasenschutz, Handschuhe und vor allem die Handhygiene als Präventionsmaßnahme.

Abstract

Infections by multi resistant bacteria (MRSA, VRE, ESBL) represent an increasing problem for health care settings. Endoscopic examination < than 12 months ago is regarded as one possible transmission route among HA-MRSA infections. Bronchoscopies and percutaneous endoscopic gastrostomies have a considerable contamination risk because of the oro-pharyngeal bacterial colonisation. Multi resistant bacteria, and like MRSA have a long environmental persistence. Screening for MRSA infected patients, decontamination and special contact precautions, universal gloving and in particular hand hygiene, are necessary preventive methods to avoid contaminations in endoscopic settings.
