Zusammenfassung

Die MDCT und MRT besitzen einen festen Stellenwert in der Dünndarmdiagnostik. Der Beitrag liefert eine Übersicht über Technik und Indikationsspektrum sowie Vor- und Nachteile dieser Verfahren. Anhand einer Auswahl relevanter Dünndarmerkrankungen aus dem Formenkreis Entzündung und Tumoren sowie gastrointestinaler Blutung werden charakteristische bildmorphologische Befunde sowie Tipps und Tricks vorgestellt und in alltagstaugliche diagnostische Algorithmen eingebunden.

Abstract

MDCT and MRT are valuable methods for diagnosing small bowel diseases. This article provides an overview of the techniques, indication spectrum, and advantages and disadvantages. Characteristic morphological image findings and diagnosing tips and tricks are explained on the basis of a selection of relevant small bowel diseases including inflammation, tumours and gastrointestinal bleeding.

  • CT und MRT sind in der Dünndarmdiagnostik vor allen dann von Bedeutung, wenn endoskopische Verfahren an ihre Grenzen stoßen. Im Vordergrund stehen die direkte Visualisierung der Darmwand und ihrer unmittelbaren Umgebung und die Darstellung des arteriellen Zu- und venösen Abstroms.

  • Drei klinischen Ausgangssituationen für Dünndarmpathologien stehen entsprechende radiologische Untersuchungsverfahren gegenüber: Chronisch entzündliche Darmerkrankungen (CED), gastrointestinale Tumoren, Malabsorption und Subileus werden mit CT- und MRT-Enteroklysma/‑Enterografie untersucht, eine unspezifische abdominelle Symptomatik mit der Standard-CT des Abdomens und eine gastrointestinale Blutung oder Mesenterialischämie mit der biphasischen CT-Angiografie.

  • Die normale Darmwand ist bei guter Distension nicht dicker als 2 mm. Bei einer objektivierbaren Darmwandverdickung lassen sich für die CT 7, für die MRT insbesondere bei Morbus Crohn 3 Befundmuster grundsätzlich abgrenzen.

  • Diese Anzahl bildmorphologisch erfassbarer Veränderungen ist gering gegenüber der breiten Differenzialdiagnose von Dünndarmerkrankungen. Mit der Beurteilung der Darmwand (Typen, Länge der betroffenen Segmente, Grad der Wandverdickung), extramuraler Strukturen, anderer Organe und der Berücksichtigung klinischer Angaben ist jedoch eine artdiagnostische Eingrenzung oft möglich.
