Kernaussagen

Systemische (Auto-)Immunerkrankungen mit Leberbeteiligung

  • Insbesondere die hepatische Manifestation einer Sarkoidose und die IgG4-assoziierte Erkrankung (IgG4-assoziierte Cholangitis) müssen differenzialdiagnostisch bei cholestatischen Leberwerterhöhungen bedacht werden. Ihr systemischer Charakter kann hilfreich für die Diagnose sein.

Autoimmune Hepatitis

  • Die Diagnose einer AIH ist schwierig und muss differenzialdiagnostisch bei unklaren Leberwerterhöhungen und unklarer Leberzirrhose bedacht werden.

  • Die Therapie der AIH besteht oft in lebenslanger Immunsuppression.

Primär biliäre Zirrhose

  • An eine PBC sollte bei älteren Frauen mit Cholestase gedacht werden. Eine AP-Erhöhung und der Nachweis von antimitochondrialen Antikörpern (AMA) sind ausreichend für die Diagnosestellung.

  • Die Therapie mit Ursodeoxycholsäure (UDCA) kann den Progress der PBC häufig einschränken.

Primär sklerosierende Cholangitis

  • Die PSC betrifft vorrangig jüngere Männer und ist mit chronisch entzündlichen Darmerkrankungen assoziiert.

  • Lokale Komplikationen der Gallenwege bedürfen oft einer endoskopischen Intervention. Eine Therapie zur Reduktion der Progression der PSC existiert nicht.

Sekundäre autoimmune Hepatitis (Overlap-Syndrom)

  • Eine sekundäre autoimmune Hepatitis wird als Variante der klassischen autoimmunen Lebererkrankungen angesehen.

  • Die hepatitische Komponente wird mit Steroiden und die cholestatische Komponente mit UDCA therapiert.
