Automatisierter Brustultraschall vs. handgeführter Brustultraschall: BI-RADS-Einstufung, Untersuchungsdauer und Erlebnisqualität der Untersuchung

Zusammenfassung

Ziel: Der automatische Brustultraschall (ABUS) ist eine potenziell wertvolle Ergänzung zur alleinigen Mammografie im Screening von Mammakarzinomen. Die Zuverlässigkeit und Interobserver-Variabilität der Einstufung nach BI-RADS im Vergleich zum handgeführten Ultraschall (US) sowie Untersuchungsdauer und Erlebnisqualität bei ABUS-Untersuchungen wurde bisher nur in sehr wenigen Studien untersucht. Material und Methoden: In einer prospektiven Studie wurden 148 Brüste von 76 Patientinnen sowohl mit handgeführtem US als auch mit ABUS untersucht. Die ABUS-Datensätze wurden von 2 Befundern getrennt ausgewertet. Die Erlebnisqualität der Untersuchungen wurde durch einen standardisierten Fragebogen erhoben. Ergebnisse: Die Übereinstimmung der BI-RADS-Einstufung ABUS beider Befunder war sehr gut (κ = 0,750), die Übereinstimmung zum handgeführten US moderat (κ = 0,592 bzw. κ= 0,518). Die Sensitivität des ABUS lag bei Befunder 2 bei 75 %, bei Befunder 1 und dem handgeführten US bei 87,5 %, die der Mammografie bei 83 % für die Diagnose eines Mammakarzinoms. Die Untersuchungen wurden von 50 Patientinnen (66 %) beim handgeführten US und 49 der Patientinnen (64 %) bei ABUS als völlig schmerzfrei angegeben, von 20 Patientinnen (26 %) bei handgeführtem US und 19 Patientinnen (25 %) bei ABUS als gering schmerzhaft und von 6 Patientinnen (8 %) bei handgeführtem US und 8 Patientinnen (10 %) bei ABUS als erträglich schmerzhaft. Schlussfolgerung: ABUS-Untersuchungen zeigen bezüglich der US BI-RADS-Einstufung eine geringe Interobservervariabilität. Die Sensitivität der ABUS Untersuchung und des handgeführten US unterscheidet sich nicht signifikant.

Abstract

Purpose: Automated breast ultrasound (ABUS) is a potentially valuable adjunct to mammography in breast cancer screening. The reliability and the inter-observer variability in the BI-RADS classification, compared to handheld ultrasound (US), as well as the duration of the examination and patient comfort have only been investigated in a limited number of papers to date. Materials and Methods: In a prospective study, we examined 148 breasts of 76 patients with handheld US and ABUS. The ABUS data were evaluated separately by two investigators. Patient comfort was assessed using a standardized questionnaire. Results: The inter-observer agreement for the BI-RADS classification among the two observers using ABUS was high (κ = 0,750), the agreement with handheld US was moderate. The sensitivity in the detection of breast cancer was 87.5 % for handheld US and 75 % for the ABUS evaluation by observer 1. The sensitivity was 87.5 % for the ABUS evaluation and 83 % for mammography by observer 2. The ABUS examination was rated as completely painless by 64 % of the patients. 25 % of the patients indicated minor pain, and 10 % indicated moderate pain. Handheld US was rated as completely painless by 66 % of the patients. 26 % of the patients indicated minor pain, and 8 % indicated moderate pain. Conclusion: ABUS examinations focusing on the BIRADS classification have low inter-observer variability, compared to handheld US. The sensitivity of ABUS did not differ significantly from handheld US.
