CRPS – Complex Regional Pain Syndrome – Eine aktuelle Übersicht über Klassifikation und Klinik

Zusammenfassung

Das komplexe regionale Schmerzsyndrom Typ I (CRPS I) – früher auch als Morbus Sudeck oder als sympathische Reflexdystrophie (SRD) bezeichnet – beschreibt einen Symptomen-komplex mit chronischen, oft schwer beherrschbaren Schmerzen, autonomen, sensomotorischen und trophischen Störungen. Auslöser sind zumeist Traumen oder operative Eingriffe, eine psychische Prädisposition ist umstritten. Goldstandard der Diagnostik ist eine umfassende Anamnese und klinische Untersuchung. Radiologische Verfahren dienen der Diagnosesicherung. Eine intensive multimodale Therapie ist unabdingbar. Der vorliegende Artikel beleuchtet die Klassifikation und Klinik des CRPS.

Abstract

Complex regional pain syndrome type I (CRPS I) – formerly named as Sudeck's atrophy or reflex sympathetic dystrophy (RSD) – describes a complex of symptoms with chronic, poorly controllable pain, autonomic, sensomotoric and trophic alterations. It is mainly caused by trauma or surgery, a mental predisposition is disputed. A profound medical history together with a meticulous clinical examination is still the diagnostic “gold standard”. Radiological modalities alone can secure the diagnosis. The initiation of an intensive multimodal therapy is mandatory. The present article highlights the classification and clinical presentation of the disease.
