Kernaussagen

  • Das wichtigste bildgebende Verfahren zur Frühdiagnostik des Mammakarzinoms ist nach wie vor die Röntgenmammografie. Sie ist das einzige Verfahren, für das nachgewiesen wurde, dass sein Einsatz im qualitätsgesicherten Screening die Brustkrebsmortalität senken kann.

  • Die digitale Tomosynthese der Brust (DBT) liefert auf der Basis einer begrenzten Anzahl von Einzelaufnahmen mit unterschiedlichem Projektionswinkel überlagerungsfreie Schichtaufnahmen und kann so die unerwünschte Maskierung durch überlagerte Strukturen beseitigen.

  • Für die Umsetzung der Tomosynthese ist insbesondere erforderlich, dass der Detektor eine hohe detektive Quantenausbeute aufweist, um Einzelaufnahmen mit geringer Dosis zu ermöglichen, und dass seine Auslesetechnik eine schnelle Bildfolge zulässt. Viele Parameter mit Einfluss auf die Bildqualität der DBT sind noch in der Diskussion, es hat sich noch kein Standard herausgebildet.

  • Ob die DBT als Alternative zur digitalen Mammografie gesehen werden kann oder nur ein additives Untersuchungsverfahren im Rahmen der Abklärungsdiagnostik darstellt, ist noch nicht entschieden. Kriterien zur Beurteilung dieser Frage sind u. a. Sensitivität und Spezifität, Kosten, Strahlenexposition, Flexibilität der Einsatzmöglichkeiten und Einfluss auf den klinischen Arbeitsablauf.
