Epidemiologie und Diagnostik

  • Da Adenokarzinome des Magens und des gastroösophagealen Übergangs weltweit zu den häufigsten tumorbedingten Todesursachen gehören, kommt den perioperativen, adjuvanten und palliativen Therapiestrategien, die im Laufe der letzten Jahre entwickelt wurden, eine große Bedeutung zu.

  • Die neuen Konzepte erfordern hinsichtlich Diagnostik und Therapie eine intensive interdisziplinäre Zusammenarbeit zwischen Hausärzten, Gastroenterologen, Chirurgen und den spezialisierten Tumorzentren.

  • Während die Gastroskopie nach wie vor den Goldstandard zur Diagnosesicherung darstellt, haben vor allem EUS und (Mini-)Laparoskopie zur Beurteilung der Lymphknoten und des Peritoneums an Bedeutung gewonnen, da nur das genaue T- und N-Staging die spätere zielgerichtete Therapie ermöglicht.

Therapie

  • Unter Beachtung der strengen Indikationsstellung kann ein Magenfrühkarzinom vom Mukosatyp (pT1m) minimalinvasiv mittels endoskopischer Mukosaresektion (EMR) oder laparoskopischer Magenwandteilresektion therapiert werden.

  • Die perioperative Chemotherapie ist der neue Therapiestandard in Europa bei großen Karzinomen und bei Lymphknotenbefall.

  • Die adjuvante Chemo- oder Radiochemotherapie nach R0-Resektion mit D1/D2-LA stellt dagegen bisher noch kein Standardverfahren dar und sollte nur bei Hochrisikopatienten erwogen werden.

  • Die neuesten Studien zur palliativen Chemotherapie des fortgeschrittenen Magenkarzinoms lassen erwarten, dass die zukünftigen Kombinationstherapien bei gleichen oder besseren Ergebnissen patientenfreundlicher sein werden als die bisherigen Standardprotokolle.
