Einleitung

Kinder mit Obstipation oder mit vermeintlich erschwerter Stuhlentleerung werden täglich in den Sprechstunden vorgestellt. Offensichtlich handelt es sich um eine harmlose Problematik, den Kindern geht es nicht schlecht, und sie sind anscheinend nicht gefährdet. Eltern werden mit einer kurzen Beratung und der Ermahnung, für mehr Flüssigkeitszufuhr und eine gesunde Ernährung zu sorgen, nach Hause geschickt.

Leider begünstigt bei Kleinkindern das inkonsequente Management einer akuten Obstipation die Entwicklung einer funktionellen chronischen Obstipation. Diese wiederum bedingt mit all ihren Belastungsfaktoren ein erhebliches Risiko für eine Beeinträchtigung der Lebensqualität und der gesunden Entwicklung des Kindes. Auch die Prognose einer chronischen Obstipation ist durchaus schwierig: Eine niederländische Untersuchung wies nach, dass auch nach der Pubertät noch bei etwa 30 % der Patienten obstipationsbedingte Beschwerden bestehen [1]. Eine Obstipation muss also ernst genommen und entsprechend diagnostisch und therapeutisch angegangen werden. Die Qualität der Versorgung obstipierter Kinder zu steigern, stellt eine schwierige Aufgabe dar [2].

Eine weitere Herausforderung im Umgang mit obstipierten Kindern besteht darin, durch einfache anamnestische und klinische Werkzeuge die wenigen Kinder herauszufinden, deren Obstipation eine organische Ursache hat. Der überwiegenden Mehrheit mit funktioneller Stuhlentleerungsstörung sollten so unnötige, invasive diagnostische Maßnahmen erspart werden.

Diagnostik und Therapie der Obstipation gehören primär in die Hände der niedergelassenen Pädiater; der Spezialist ist nur für die schwierigen Fälle erforderlich. Im folgenden Artikel soll ein praxisorientierter Überblick über alle Aspekte der Obstipation und der obstipationsassoziierten Stuhlinkontinenz bei Kindern und Jugendlichen gegeben werden und die Leserinnen und Leser in die Lage versetzt werden, Kinder mit Obstipation kompetent zu diagnostizieren und mit modernen Methoden erfolgreich zu behandeln.
