Leber

  • Die Leber ist das mittels kontrastverstärkter Ultraschalltechnik am häufigsten untersuchte Organ. CEUS erleichtert die Differenzierung zwischen fokal nodulärer Hyperplasie und hepatozellulärem Adenom sowie die Identifizierung fokaler Mehr- und Minderverfettungen.

  • Die sonografische Detektionsrate von Tumoren kann durch den Einsatz von CEUS deutlich gesteigert werden. Ein hypervaskularisierter Tumor ≥ 20 mm in einer zirrhotisch umgebauten Leber gilt bei typischem Anreicherungsverhalten auch ohne bioptische Sicherung als HCC.

Pankreas

  • CEUS kann wertvolle Beiträge zur Differenzierung von Raumforderungen des Pankreas, insbesondere soliden Tumoren, Pseudozysten und zystischen Neoplasien leisten. Bei der akuten Pankreatitis wird die Abgrenzung und Verlaufskontrolle von Nekrosearealen erleichtert.

Weitere Organe

  • Auch die CEUS-Untersuchungen von Milz (Abklärung von Raumforderungen und Hyposplenie) und Niere (v. a. Abklärung von Nierenformvarianten und komplexen Nierenzysten, insbesondere Abgrenzung gegenüber Nierenkarzinom) gehören zu den etablierten Methoden.

  • Die kontrastverstärkte Lymphknotensonografie hat sich dagegen bisher nicht als generell akzeptierte Methode etablieren können.

  • Bei der Diagnostik des Gastrointestinaltrakts ist CEUS hilfreich zur Abklärung entzündlicher Aktivität und von Ischämieformen, zur Stenosediagnostik wird der Einsatz kontrovers diskutiert.
