Zusammenfassung

Der akute Thoraxschmerz zählt zu den häufigsten Leitsymptomen, mit denen sich Patienten in einer Notfallambulanz vorstellen. Hauptaufgabe der initialen Diagnostik ist der Nachweis bzw. Ausschluss einer potenziell akut lebensbedrohlichen Ursache. Die Verfahren mit dem höchsten Stellenwert sind das konventionelle Röntgenbild des Thorax und die Computertomografie (CT). Die Magnetresonanztomografie (MRT) spielt aktuell in der klinischen Routine aufgrund der eingeschränkten Verfügbarkeit und Untersuchungsdauer eine untergeordnete Rolle.

Im folgenden Artikel wird systematisch auf die akut lebensbedrohlichen Ursachen des akuten Thoraxschmerzes und dessen radiologische Diagnostik eingegangen. Es werden die bildgebenden Verfahren, deren technische Durchführung sowie die Bildinterpretation erläutert.

Abstract

Acute chest pain is one of the main symptoms leading to a consultation of the emergency department. Main task of the initial diagnostic is the confirmation or exclusion of a potentially life threatening cause. Conventional chest x-ray and computed tomography are the most significant techniques. Due to limited availability and long examination times magnetic resonance tomography rather plays a limited role in routine clinical workup.

In the following paper we will systematically review the radiological diagnostic of the acute life threatening causes of chest pain. Imaging modalities, technical aspects and image interpretation will be discussed.

  • Hauptaufgabe der initialen Diagnostik bei akutem Thoraxschmerz ist der Nachweis respektive Ausschluss einer potenziell akut lebensbedrohlichen Ursache.

  • Nach Eingrenzung der Verdachtsdiagnosen anhand von Anamnese, klinischer Untersuchung, Laborresultaten und EKG wird die radiologische Diagnostik für die weitere Abklärung eingesetzt.

  • Die Verfahren mit dem höchsten Stellenwert sind das konventionelle Röntgenbild des Thorax und die CT.

  • Lassen sich die „klassischen 3” Differenzialdiagnosen – akutes Koronarsyndrom, akutes Aortensyndrom und Lungenarterienembolie – nicht weiter einschränken, wird eine CT des Thorax nach „Triple-rule-out”-Protokoll empfohlen.

  • Konnten die akut lebensbedrohlichen Ursachen des Thoraxschmerzes ausgeschlossen werden, werden die nicht akut lebensbedrohlichen Ursachen abgeklärt, sofern sie nicht bereits im Rahmen der vorangegangenen Diagnostik erfasst wurden.
