Akutbehandlung der Opioidabhängigkeit

Kernaussagen

• Bei der Akutbehandlung von Opioidabhängigkeit muss die Intoxikation vom Opioidentzug unterschieden werden. Die schwere Opioidintoxikation bedarf intensivmedizinischer Behandlung und der Gabe eines Opioidantagonisten wie Naloxon.

• Der Opioidentzug kann opioidgestützt oder „kalt” durchgeführt werden. Beim „kalten” Entzug werden Entzugszeichen symptomatisch mit Nichtopioiden wie Clonidin und Analgetika gemildert.

• Durch Gabe des Opioidantagonisten Naltrexon kann der Entzug beschleunigt durchgeführt werden.

• Beim opioidgestützten Entzug wird der Abhängige nach Auftreten von Entzugszeichen zunächst mit einem Opioid in einen stabilen Zustand gebracht, in welchem er weder Entzugssymptome aufweist noch intoxikiert ist. Dann erfolgt die Abdosierung über Tage bzw. Wochen.

• Als Mittel der Wahl haben sich für den Heroinentzug Methadon und Buprenorphin erwiesen.

• Bei Entzug von Opioidanalgetika bietet sich die Abdosierung des entsprechenden Analgetikums an.

• Neben der medikamentösen Unterstützung des Entzuges sollten psycho- und soziotherapeutische Ansätze genutzt werden, um die psychosoziale Situation des Abhängigen möglichst so zu gestalten, dass die Postakutbehandlung nachfolgen kann.

• Die aktuellen Rückfallraten der Abhängigen sind auch nach Einführung des opioidgestützten Entzugs unverändert hoch.
