Acne conglobata infantum – erfolgreiche Behandlung mit Isotretinoin per os

Zusammenfassung

Die selten vorkommende Acne infantum betrifft Säuglinge und Kleinkinder. Pathogenetisch sind endokrinologische Ursachen zu bedenken und mittels Hormonanalyse sowie ggf. sonografischer Untersuchung der Nebennierenrinde auszuschließen. Hier wird über ein 2 3/12 Jahre altes männliches Kleinkind berichtet, bei dem knotige Hautveränderungen mit Pus-Entleerung an beiden Wangen auftraten. Nach Versagen mehrerer systemisch applizierter Antibiotika sowie Prednisolon (Suppositorien) kam Isotretinoin per os in einer Dosierung von 0,625 mg/kg Körpergewicht und Tag erfolgreich zum Einsatz. Obwohl nicht für Kinder unter 12 Jahren zugelassen, ist Isotretinoin eine sichere Therapieoption für Acne conglobata infantum.

Abstract

Acne infantum is a rare occurring dermatosis of neonates and infants. Pathogenesis includes endocrine causes, which should be excluded by hormone analysis and ultrasound scan of adrenocortical glands. A 2 3 / 12 year old boy is described who developed nodular lesions with pus emptying of both cheeks. Oral antibiotics and prednisolone (suppositoria) failed. Isotretinoin was given with a dosage of 0.625 mg per kg body weight daily. This treatment was successful, lesions disappeared after 4 œ months therapy. Although isotretinoin is not approved for infants below 12 years this retinoid should be considered as a safe and effective treatment option in acne conglobata infantum.
