Zusammenfassung

Ein pulssynchroner Tinnitus ist durch durch ein rhythmisches Geräusch gekennzeichnet, das vom Patienten im Ohr wahrgenommen wird, und entweder dem Pulsschlag oder der Atmung angepasst ist. Anders als beim klassischen Dauerpfeifton im Ohr lassen sich beim pulssynchronen Tinnitus oft krankhafte, meist vaskuläre Ursachen in der Bildgebung feststellen. Dieser Beitrag soll zeigen, welche Ursachen es gibt, wie man sie darstellt und klar machen, dass es gefährliche Ursachen für ein pulssynchrones Ohrgeräusch gibt, die behandelt werden müssen.

Abstract

A pulse synchronous tinnitus is characterized by a rhythmic sound that is in sync with the patient heartbeat or respiration and can be perceived by the patient as a bruit. Different from the more common classic and continuous ear sound, with pulse synchronous tinnitus one may often discover a pathological neovascular condition that can be diagnosed with medical imaging. This review covers the different conditions leading to pulse synchronous tinnitus and provides indications requiring preventive and active treatment to avoid potential but serious sequela for the patient.

  • Pulssynchrone Ohrgeräusche entstehen ohrfern – anders als der klassische „Pfeif”-Tinnitus – und haben fast immer eine vaskuläre Ursache.

  • Diagnostisch sind oft alle Routineverfahren der Radiologie – also CT und MRT mit ihren Angiografievarianten und die Katheterangiografie nötig, um die Ursache zweifelsfrei aufzudecken.

  • Wichtig dabei ist, gefährliche Ursachen für das pulssynchrone Ohrgeräusch zügig zu diagnostizieren, um eine Hirnblutung zu verhindern.

  • Endovaskuläre Behandlungstechniken haben heute bei der Therapie der vaskulär bedingten Ohrgeräusche sehr große Bedeutung.
