Was bestimmt die Compliance von Patienten?

Die Compliance des Patienten stellt in der Zahnmedizin ein relevantes Problem dar. Non–Compliance kann sich z. B. in mangelnder Mundhygiene, unregelmäßigen Zahnarztbesuchen, Versäumen von Terminen oder mangelnder Mitarbeit in der Behandlung zeigen. In der Parodontitistherapie ist die Recall–Compliance des Patienten von besonderer Bedeutung.

Eine Vielzahl von Faktoren beeinflusst die Mitarbeit des Patienten in der Behandlung. Weibliche Patientinnen zeigen tendenziell eine bessere Compliance, hinsichtlich des Alters sind die Befunde bislang widersprüchlich. Während der Bildungsstand keinen Einfluss zu haben scheint, sind ein höherer sozioökonomischer Status und eine Privatversicherung eher förderlich. In der Parodontologie geht eine operative Behandlung mit einer besseren Compliance einher als eine konservative; sie sinkt dagegen bei längerer Behandlungsdauer, höheren Kosten und häufigem Behandlerwechsel. Essenziell ist die Qualität der Zahnarzt–Patient–Beziehung sowie auf der Seite des Patienten stabile Beziehungen und eine positive Einstellung der Bezugspersonen zur Behandlung. Negativ wirken sich psychische Störungen, wie Angst und Depression, ein negatives Selbstbild, belastende Lebensereignisse, Stress, Rauchen und eine negative Einstellung zur Mundhygiene aus.

Es ist für den Zahnarzt von Wichtigkeit, diese Risikofaktoren zu kennen, um auf mögliche Compliance–Probleme seines Patienten optimal eingehen zu können.
