Zusammenfassung
Durch die Koronarographie ist es möglich, die Hämodynamik beim Syndrom von Bland-White-Garland zu untersuchen; es erscheint physiologisch richtiger, eine Einteilung nach dem Durchströmungstyp
etwa im Sinne eines Koronarien-Steal-Syndroms zu treffen und die Bezeichnungen infantiler
oder Erwachsenentyp fallenzulassen. Ein eigener Fall mit Stromumkehr im linken Ramus
circumflexus und isoliertem Abgang des Ramus interventricularis aus der Aorta wird
beschrieben.
Summary
The haemodynamics of the Bland-White-Garland syndrome can be investigated by coronary
arteriography; physiologically it is more appropriate to consider the lesion as a
coronary steal syndrome and to abandon the classification into infantile and adult
types. The authors describe a case with reversed flow in the left circumflex artery
and with isolated origin of the interventricular artery from the aorta.
Résumé
La coronarographie permet l'étude de l'hémodynamisme dans le syndrôme de Bland-White-Garland.
Il semble plus exact au point de vue physiologique de subdiviser ce syndrôme selon
le type du courant et d'abandonner les dénominations de type infantile et de type
adulte. Description d'un cas avec renversement du sens de la circulation dans l'artère
circonflexe gauche, le rameau interventriculaire prenant son départ directement et
isolément de l'aorte.
Resumen
Por medio de la coronarografía es posible investigar la hemodinámica en el síndrome
de Bland-White-Garland ; parece fisiológicamente más correcto encontrar una clasificación según el tipo de
irrigación sanguínea, acaso en el sentido de un síndrome Steal de coronarias y dejar
las denominaciones: tipo infantil o adulto. Se describe un caso particular con inversión
de corriente en la rama circunfleja izquierda y evacuación aislada de la rama interventricular
de la aorta.
