Angiographische Probleme und Möglichkeiten^*

Zusammenfassung

Voraussetzung für die komplikationslose Arteriographie und den Arterienkatheterismus ist die sichere Lage der Punktionskanüle im Gefäßlumen. Unter Hinweis auf die technischen Schwierigkeiten wird eine einfache Arterienpunktionskanüle mit flexiblem Mandrin vorgeschlagen. Für den Zweck des Arterienkatheterismus nach Seldinger wird diese Kanüle in ihrer Funktion erweitert. Außerdem wird ein einfaches Arterienkompressorium zur Vermeidung von Hämatomen nach beendigter Untersuchung bei Punktion mit großkalibrigen Kanülen angegeben. Die beste Gewähr für einen komplikationslosen Verlauf dieser noch zu wenig gebräuchlichen röntgenologischen Untersuchungsverfahren bietet eine korrekte Punktionstechnik, verbunden mit geeignetem Instrumentarium. Unter diesen Voraussetzungen kann die Arterienpunktion und der Arterienkatheterismus bei den bereits hierfür bestehenden Indikationen dem Röntgenologen allgemein empfohlen werden und muß keineswegs nur dem Spezialisten vorbehalten bleiben.

Summary

Satisfactory position of the canula in the lumen of the vessel is essential for the avoidance of complications during arteriography or arterial catheterisation. In view of the technical difficulties, a simple canula for arterial puncture with flexible stilette is described. This canula has a wider function in the performance of arterial catheterisation by the Seldinger technique. Further, a simple arterial compressor has been designed in order to avoid haematomas after puncture with wide-bore canulae. The best way to avoid complications during this procedure, which is still insufficiently used, is a correct technique of vessel puncture and the use of suitable instruments. Provided these conditions are fulfilled, arterial puncture and catheterisation can be carried out by all radiologists when indications for their use are present, and need not be confined to the specialist.

Résumé

Pour qu'une artériographie ou qu'un cathétérisme artériel ne présente pas de complications, il faut que l'aiguille soit bien située dans la lumière de l'artère. En démontrant les difficultés techniques l'auteur propose l'utilisation d'une canule simple de ponction artérielle avec un mandrin flexible. La canule peut être utilisée également pour le cathétérisme artériel d'après Seldinger. L'auteur indique encore un compresseur artériel simple permettant d'éviter la formation d'hématome en retirant les grosses canules è la fin de l'examen. La meilleure garantie d'une évolution sans complication de ce procédé radiologique encore trop peu employé nous est donnée par une technique de ponction correcte avec des instruments appropriés. Si l'on tient compte de ces remarques et si l'on observe les indications précises la ponction artérielle et le cathétérisme artériel peuvent être effectués couramment par le radiologiste, ils ne sont pas nécessairement l'apanage d'un spécialiste.

Condición previa para una arteriografía o cateterismo arterial sin complicaciones depende de la posición segura de la aguja de punción en la luz vascular. Después de señalar las dificultades técnicas se propone una cánula de punción arterial simple con mandril flexible. Con el fin de realizar el cateterismo arterial según Seldinger, se amplía en su función este dispositivo. Además se indica un sencillo compresor de arterias para evitar la formación de hematomas una vez finalizado el examen por punción con aguja de calibre grande. Lo mejor para evitar complicaciones con esta técnica aún demasiado poco difundida, es una correcta técnica de punción asociada a instrumental adecuado. Bajo estas condiciones, la punción arterial y el cateterismo arterial, con sus respectivas indicaciones, pueden ser recomendadas al radiólogo en general y no consideradas de recurso exclusivo del especialista.
